Personal Medical Organizer
A few years ago, someone in the mental health community asked me to speak to a group of clients and their family members about effective ways of organizing personal medical information, a presentation I have now delivered to several local groups.
While researching the topic, I learned about the importance of keeping track of one’s own medical history, including symptoms, prescription drugs, treatments, and so on. Keeping this information together in an organized fashion makes things much easier if you have to apply for any type of disability income or other related services, or if you have to see a new healthcare provider, whether it be a specialist, emergency room staff, or other professional. It also allows you to monitor whether your condition is getting better or worse, and at what rate, as it’s not always easy to remember how long you’ve been experiencing a certain symptom or when you stopped waking up with a headache.
I discovered a number of excellent products available for this purpose, but some of them are not available in Canada, and others I felt were too expensive to recommend to individuals on a limited budget. This led me to create my own Personal Medical Organizer so I could show participants how they could set one up for themselves, and it went over so well, that I’m now offering it for sale.

Alice, the four Personal Medical Organizers shown are identical in content – the three shown lower on the page are just samples of cover pages we’ve custom branded for specific professional organizers to give or sell to their clients.
The organizer has sections for Personal Information, Family Medical History, Contacts, Medical History, Medications, Medical Paperwork, and Medical Expenses.
It is a digital product, so you can print as many pages as you need for your family, or store each family member’s information in its own binder.
The product is designed to be easy to use, and includes an instruction sheet outlining how to assemble and use your binder.

This is a very good organization tool for those of us that like to use pen and paper as opposed to Blackberrys and iPhones, laptops, etc.
I like that the pages are binder sized and pretty easily be customized to work for all family members.
My recommendation would be to change the “Medical History” to “Medical Visits” and have larger blocks of space in which to write.
An immunization record would also be good, but not completely necessary as many doctors give those out and they can simply be kept with the notebook.
I LOVE that this is a PDF file that can be kept on the computer because when I use up a page, I can simply reprint a copy.

My thoughts when developing this were to keep it simple, so that medical visits, immunizations, hospital stays, etc. would all be included under Medical History, so it’s great to have some input from a medical professional. I’ll consider some of those changes in future editions of the Personal Medical Organizer.

I would like to see, on line, the inside of each of your three medical organizers so that I can choose which would be the best for my family. Topics, organization, is there a place for insurance papers, etc. are things I am interested in. Also, how many people in the family can be accommadated in one book? What is the ease of use?
Is it organized by month, topic, family member?
